ID Documents
身份证件与 I-693 表格
请携带有效政府签发的带照片身份证件，例如护照、驾照或州 ID。如已准备 Form I-693，请一并携带；请勿提前签名，请在医生或诊所工作人员指导下签署。
Please bring a valid government-issued photo ID, such as a passport, driver’s license, or state ID. If you have prepared Form I-693, please bring it with you. Please do not sign the form until instructed by the doctor or office staff.
疫苗记录
Vaccination Records
请携带可取得的既往疫苗记录，包括儿童时期、学校、工作、药房或其他医生处的疫苗记录。如有翻译件，也请一并携带。
Please bring any available vaccination records, including records from childhood, school, work, pharmacy, or other medical offices. Translation is helpful if available.
既往 TB 或胸片资料
Prior TB or Chest X-ray Records
如曾有结核筛查异常、阳性 TB test、胸片异常、结核治疗或潜伏结核治疗史，请携带相关检查及治疗资料。
Please bring prior TB test results, chest X-ray records, TB treatment records, or latent TB treatment records if applicable.
Relevant Medical Records
相关医学资料
如有重大疾病、手术、住院、长期用药记录、精神健康相关病史，或其他重要治疗记录，请携带可取得的相关资料。
Please bring available records for major medical conditions, surgeries, hospitalizations, long-term medications, mental health history, or other important treatment history if applicable.

医生体检与资料审核

Medical Exam & Record Review
医生将进行体检，并审核疫苗记录和相关医学资料。
The doctor will perform the medical exam and review vaccination records and relevant medical documents.
必要检查与疫苗评估

Required Tests & Vaccination Assessment
根据 USCIS/CDC 当前要求及个人情况，安排必要检查、疫苗评估或补充资料。
Required tests, vaccination assessment, or additional documentation may be arranged based on current USCIS/CDC requirements and individual circumstances.

领取 I-693 密封信封
Receive Sealed Form I-693 Envelope
完成所有要求后，诊所将准备 I-693 密封信封。请勿自行打开密封信封。
After all requirements are completed, our office will prepare the sealed I-693 envelope. Please do not open the sealed envelope.
